ACT Global is an international network of clinicians, researchers, and health systems focused on improving care for people living with rheumatic heart disease.
The network uses data, structured collaboration, and evidence-based practice to strengthen diagnosis, follow-up, secondary prevention, and long-term outcomes.
Rheumatic heart disease is manageable, but many health systems still lack the infrastructure needed to identify patients early, keep them in care, and monitor outcomes over time. ACT exists to support that infrastructure.
ACT was developed with global partners as a scalable case management solution for RHD care. It gives participating clinical teams a structured way to manage patient data, strengthen follow-up, and use evidence to improve care across diverse settings.
ACT Global brings this work into an international network. By connecting clinicians, researchers, health systems, and partner institutions, ACT Global supports shared learning, quality improvement, and coordinated action against a disease that continues to affect some of the world’s most under-resourced communities.
Today, ACT Global is focused on helping sites integrate ACT into clinical workflows, improve data quality, support long-term patient management, and build the evidence needed to reduce the burden of RHD.
ACT Global builds on more than a decade of structured RHD care, implementation learning, and platform development.
Uganda establishes one of the first RHD registries in sub-Saharan Africa.
Uganda joins the first international RHD research registry, REMEDY.
The registry converts to an electronic platform on REDCap.
A grant from the American Heart Association supports further enhancement.
ACT (Active Community Case Management Tool) is built and piloted as a mobile registry.
ACT is adopted as the National RHD Registry at the Uganda Heart Institute.
ACT expands to regional RHD sites and supports district-wide RHD control programs in partnership with the Ugandan Ministry of Health.
ACT Global is founded to address the global need for coordinated RHD care infrastructure.
ACT is revamped with input from ACT Global partners to better support clinical workflows, patient management, data quality, and site-level implementation.
The updated ACT is relaunched for ACT Global partners.
ACT Global partners begin launching ACT at their sites and integrating the platform into local workflows.
